Mount Auburn Professional Services at Belmont Medical
has established a comprehensive diabetes center of excellence. Under one roof patients will be able to be managed by an experienced team which includes three diabetologists, a nurse practitioner, a diabetes educator and with a link to dieticians, podiatrists and ophthalmologists.
DIABETES QUALITY OF CARE GOALS
HbA1c testing : At least 2 tests per year
HbA1c poor control >9% : Most recent test should not exceed 9%. Goal is <7%
LDL testing : At least 1 test per year
LDL Control <100mg/dl : Most recent LDL should be <100
Blood Pressure <140/80 : Both systolic and diastolic below target eg 139/79
Eye Exam : 1 retinal exam with eye doctor per year
Kidney screening for microalbumin : At least one urine microalbumin test per year
